When a patient says a required document has not reached the next office, coordination begins with facts: what was sent, which channel was used, and who receives it. A care coordinator can connect the sender and recipient through approved processes. Interpreting clinical content or deciding whether a document is sufficient belongs to the authorized team.
Noah says, “The clinic says it never got the form my other office sent.” The coordinator replies, “I can help identify the sending office and the receiving team, then ask them to confirm the operational status through their approved channels.
